If this is your first arrhythmia consultation, your initial visit will include an evaluation based on current symptoms or conditions and a thorough compilation of your medical history. Patients should bring all medical records related to their diagnosis.

Being able to provide the following information will expedite the scheduling process:
- Patient's name, address, and telephone number
- Patient's date of birth
- Patient's social security number
- Name and contact information of patient’s primary care physician or cardiologist
- Patient’s insurance information
- If applicable, patient’s manufacturer-issued pacemaker or defibrillator card information
