Refer Your Patient

Refer a Patient to the Emory Adult Congenital Heart Center (EACH)

The EACH Center specializes in the treatment of adult patients with moderate and complex congenital heart defects (CHDs). The Center features a multidisciplinary management team that includes both adult and pediatric cardiologists, congenital cardiac surgeons, electrophysiologists, interventional cardiologists, nurse practitioners (NPs), nurses, social workers and echocardiographers. We also work with collaborative partners who have a special interest and expertise in managing adults with CHDs from a range of other specialties.

To refer a patient, you can call 404-778-5036 and speak with one of our administrative assistants. You will also need to fill out our Referral Form and Document Checklist (PDF 104 KB) and use it as a cover sheet to fax pertinent patient records to our office at 404-778-5035.
