Completion of the Annual Provider Patient Safety Module is one of the ways we demonstrate our commitment to and creation of a safe environment here at Emory Healthcare. All providers at EHC must complete the 2016 module by 4 p.m., Monday, December 5, 2016, to retain their privileges. Thank you for keeping Emory Healthcare a safe place for our patients and their families.

Four requirements – Deadline is 4 p.m., Monday, December 5, 2016

1. Influenza vaccination requirement: 

  • Get vaccinated. You can get vaccinated for free at an EHC flu vaccination event or Employee Health (click here for locations). For a schedule of flu vaccination events, click here, or contact Employee Health. The vaccine is mandatory for all providers, except those with documented medical contraindications or a religious exemption form signed by their religious leader. Egg-free vaccines are available at Employee Health locations; an egg allergy cannot be applied to the medical contraindications exemption. Remember: You must apply for exemption each year. NEW THIS YEAR Religious exemption forms must be signed by a religious leader/spiritual advisor in the community; in-house chaplains will no longer sign exemption forms.

    Note: If you are requesting a medical or religious exemption, don’t forget to submit your form each year. Follow the directions below to complete this requirement.

    To complete the online consent form (pre-registration) prior to getting vaccinated by Employee Health, log on to e-Vantage, then click Self Service and Influenza Vaccine Registration to complete the form. (e-Vantage is accessible via the right-hand navigation on www.emoryhealthcare.org/employee. Log on with your network ID and password.)

    To document having received the vaccination in the Annual Provider Patient Safety Module, click on the “Click here to access Provider Patient Safety Module” box at the bottom of this page.

    If you receive the vaccination outside of Employee Health, submit documentation to Cindy Hall via email at cynthia.hall@emoryhealthcare.org or fax to 404-686-1527.

    To submit a completed medical or religious exemption form (form available on the Annual Provider Patient Safety Module), email the form to Cindy Hall at cynthia.hall@emoryhealthcare.org or fax to 404-686-1527. The religious exemption form must be signed by your religious leader.

2. TB test requirement:

  • Get the TB test (tests performed within the past 12 months are permitted), unless previously tuberculin skin test (TST) positive. You can obtain a TB test at Employee Health (click here for locations) at no cost. Non-EHC-employed providers can go to any of the Employee Health offices to have the TB test placed and read. If not done at Employee Health, TST should be placed and read by an appropriately qualified individual.

  • Record your TB test by entering your TST and/or T-spot/QuantiFeron (if you have a history of a past positive TST or T-spot/QuantiFeron) results in the TB section in the Annual Provider Patient Safety Module.  

  • If you do not have your TB test placed and read at Emory Healthcare Employee Health, you must submit documentation of your TST and/or T-spot/QuantiFeron results to Cindy Hall in Employee Health at cynthia.hall@emoryhealthcare.org or fax to 404-686-1527.

3. Online curriculum requirement:

    1. Infection control test
    2. Three modules: Compliance, Emergency Medical Treatment and Active Labor Act (EMTALA), and patients with English language or hearing barriers 
  • Log in to the module. To log in, click on the “Click here to access Provider Patient Safety Module” box at the bottom of this page.

  • Take the modules and infection control test. This includes the compliance, EMTALA, and patients with English language or hearing barriers modules.

4. Immunizations review requirement:

  • Log in to the module. To log in, click on the “Click here to access Provider Patient Safety Module” box at the bottom of this page.

  • Review, add or update your immunizations.

How to know you’ve completed the module:

When you have completed all of the requirements, the overview page shows all sections as complete and offers you the option of printing a certificate (see screen capture).
