Annual Provider Patient Safety Module: October 10 - December 15, 2011
Our first priority at Emory Healthcare (EHC) is to care for the health and safety of our patients and their families. In the past, providers have been required to complete the Annual Infection Control Module. For 2011, we have modified some of the components of this module to make it easier to complete and more pertinent to our environment. To better reflect these changes, we have renamed this important requirement to the Annual Provider Patient Safety Module.
To retain your privileges, you must do two things:
1. Complete the Annual Provider Patient Safety Module by Thursday, December 15, 2011. This includes the TB screening skin test.
New! Restraints and tissue allograft management modules have been added.
2. Receive the seasonal influenza vaccination by December 15, 2011. Exceptions will be made for those with a medical contraindication or religious objection. To ensure compliance with EHC’s seasonal influenza vaccination mandate, you are required to do one of the following:
• Get vaccinated at an EHC flu vaccination event or Employee Health. For a schedule of flu vaccination events, visit the home page of the intranet (www.ourehc.org) and click the influenza vaccination schedule, or contact Employee Health at 404-728-6437.
• Submit documentation if vaccinated outside of EHC. All documentation should be sent to Cindy Hall in Employee Health via e-mail, Cynthia.hall@emoryhealthcare.org, or fax, 404-686-1527.
• Complete a medical or religious exemption form (form available on the Annual Provider Patient Safety Module)
New! EHC- and EU-employed providers only: Prior to getting vaccinated, you must complete the consent form located on e-Vantage (PeopleSoft Self Service). Once you log on, simply click Self Service, then Influenza Vaccine Registration.
