Patient/Visitor Guide
Welcome to the Patient/Visitor Guide for Emory-Children's Center. We hope you find the following information useful as a guide to your child's first visit.
First Visit
Our patient coordinators are available to schedule your child's appointment Monday through Friday between 8a.m. and 5 p.m. To schedule an Emory-Children's Center Clinic appointment, please call us a 404-778-2400.
Calling for an Appointment
Prior to calling, please have the following information:
- Child's Name
- Date of Birth
- Diagnosis or Chief Complaint
- Referring Physician's Name, Address and Phone Number
- Insurance Coverage (Name and ID)

Appointment Day
Please make sure you bring the following:
- The map with instructions on where to park (see map under Location)
- A completed New Patient Registration Form; (the form may be printed out by clicking on Pre-Registration or call our offices to have one sent to you).
- Insurance cards and referral information
- Any medical records regarding the current problems including X-rays and E.E.G. tracings.
The Clinic Visit
Please be mindful of the following:
- Parking is available at the Lowergate Parking deck pay lots. Valet parking is available at the front of Emory-Children's Center. Click here for a map.
- Handicapped parking is available in front of Emory-Children's Center
- Please allow additional time for parking.

Payment Policy and Billing
Payment is expected at the time of the clinic visit. If insurance information is provided and can be verified prior to the visit, Emory-Children's Clinic will file your child's claim, collecting only the applicable deductible, co-pay or other amount the patient is responsible for. If not, we will provide a copy of your child's bill, which you may use to file with your insurance carrier for reimbursement.
Pre-Registration Forms
Patients may expedite the process of the first visit by filling out the necessary forms in advance of the appointment. The forms may be downloaded by clicking below. Please print the forms, fill it out and bring it to the appointment or mail it to us in advance. Each patient's medical information is confidential and we cannot accept the form electronically.

Support
Emory-Children's Center provides social workers that will help patients and their families deal with all the many issues that arise during a child's illness. Each Emory- Children's Center division team will work with the patient and family to find answers for everything from financial assistance to managing psychological stress.
In addition, each division has a list of support groups that deal with issues patients and families face related to the nature of a specific illness. Click on the appropriate section below for contact information for each of the divisions.
