Patient/Visitor Guide to the Fuqua Center for Late-Life Depression

We hope the following information will help guide you through your visitor experience with our Center.  Physicians and clinical team of the Fuqua Center for Late-Life Depression are experts at the diagnosis and treatment of depression in older adults. This is not an easy task and a thorough evaluation is important. The great news is that the majority of people who suffer from depression can be effectively treated, and we are here to help! Please follow the steps below that outline what to expect on your first visit.

SCREENING

A free self-screening test is available using the Geriatric Depression Scale (Long Form) or Geriatric Depression Scale (Short Form).  The GDS is a screening tool, and is used to determine if further evaluation is needed. Please bring a completed copy with you to your appointment.

MAKING AN APPOINTMENT

You can make a new patient appointment for an outpatient evaluation at  (404) 728-6302 .  Once you have made an appointment, you will need to print, fill out and/or bring with you the following:

New Patient Information Form
Outpatient Registration form
All of your medication(s) in its prescription bottle(s)
All insurance cards (including Medicare and your secondary plan if you have one)

If you need assistance in obtaining your medical records, we will be happy to request them from your physician(s) for you.  However, in order for us to do so, you will first need to complete one copy of the Consent for Disclosure of Information form for each physician we need to contact. You may complete this form at your first visit or call us at  (404) 728-6302  for questions or concerns.

WHAT TO EXPECT YOUR FIRST VISIT

Plan on your visit taking about an hour and a half. New patient visits include a complete review of health history, medications, and blood work. Please come 30 minutes prior to your scheduled appointment and register on the 3rd Floor of the Wesley Woods Health Center, 1841 Clifton Road.

DRIVING DIRECTIONS

Exit I-85 at North Druid Hills Road
Follow North Druid Hills East to Briarcliff Road
Turn Right on Briarcliff Road
Turn Left on Clifton Road
Turn Left at first traffic light to enter Wesley Woods
Take an immediate right and follow the road to the Wesley Woods Health Center
