Emory HeartWiseSM Risk Reduction Program

Our Mission:

To provide optimal preventive and cardiac rehabilitative services, using a multidisciplinary approach tailored to the individual to assist with lifestyle changes.

To identify risk factors leading to heart disease and to provide treatment goals and strategies for reducing such risks.

To provide knowledge and skills needed to prevent heart disease recurrence, repeat procedures and hospitalization.

Who can we help?

Anyone at risk for developing coronary artery disease and those with established heart or blood vessel disease.

What services do we provide?

Cardiac Rehabilitation:  Phase 2

This service is specifically designed for you if you have experienced a heart attack, heart valve disease (repair or replacement), controlled chest pain or heart procedures, such as balloon angioplasty, stents, coronary artery bypass surgery or heart transplantation. The program is aimed at helping you return to a productive life, improve exercise capacity, self-confidence, reduce activity related symptoms and manage your heart disease risk factors. Exercise training is done with continuous electrocardiograph (ECG) monitoring. Your exercise is supervised by a medical team consisting of nurses, exercise specialists, cardiologists, and registered dietitians. This team will work together to help you in meeting your goals and reducing your risk factors for heart disease. Phase 2 cardiac rehabilitation is usually covered by most insurance plans.

Cardiac Rehabilitation:  Phase 3

Phase 3 cardiac rehabilitation is for those who have graduated from a phase 2 rehab program, or for those who would like to prevent heart disease from developing. In Phase 3 cardiac rehab you will work with our highly trained staff to develop a personalized exercise plan that will optimize your healthy lifestyle. Your exercise will be supervised by our staff and you will have an individual “case manager” to help you implement your healthy lifestyle changes. We also offer group exercise classes including aerobics, chair exercise, water aerobics, and yoga to our Phase 3 members.

Optimal Living Program

The Optimal Living Program is an eight-week jumpstart to a heart healthy lifestyle. This program is for patients without heart disease but who can benefit from personalized exercise and dietary mentoring. You will receive one on one exercise and dietary counseling and supervised exercise training with our staff. You will also receive personalized fitness and diet evaluations, individualized goals, strength training, weekly nutrition education and a life plan to continue the good changes you have started in our program.
