To schedule an appointment, the following patient information is required:

• Physician's name, address and telephone number
  
  
• Patient's name, address and telephone number; date of birth; social security number
  
  
• Diagnosis and date of diagnosis
  
  
• Prior treatment, if any
  
  
• Insurance information
