You may make an appointment at the Otolaryngology main campus (The Emory Clinic) office yourself or through a referral from your primary care physician.
Before your first appointment with one of our physicians, we suggest that you complete the medical history form below. We also request that you obtain medical records (including X-rays, CT or MRI scans, etc.) from your referring health care providers and bring them with you if possible.
These steps will help us expedite your appointment and assure we have the appropriate information needed to provide the best advice possible during your initial consultation. You may print the form, fill in all the information and return it by mail to the office of the physician you have an appointment with. Or, you may bring it with you to your first appointment. We cannot accept the form electronically due to patient confidentiality.
