Your insurance company may require a referral from your Primary Care Physician to see a specialist. If your Primary Care Physician is referring you to our clinic, please have their office fax a request (including physician and patient phone number) and pertinent supporting documents (laboratory, imaging) to (404) 686-4841. Please be sure to obtain this information prior to scheduling your appointment. If your referral and/or authorization are not up to date, our medical secretary will remind you to obtain them prior to your appointment.
