The CRM offers three inpatient programs which provide comprehensive, interdisciplinary, patient-focused services to adult populations using a combination of individual and group treatment. Case management and social services are an integral part of each program. Medical services are delivered in collaboration with a team of rehabilitation professionals. A medical director who is trained in physical and rehabilitation medicine leads each specialized program and designated team.

The Brain Injury Program: An intensive program designed for persons who have sustained a brain injury due to injury or illness. This program focuses on restoring the patient to the optimal level of independent functioning possible in an acute rehab setting along with providing education and support for the patient's primary caregivers.
The clinical program is concerned with:

  • Medical management
  • Restoration and/or adaptation of individual functional skills
  • Integration of functional skills into effective behavioral routines for daily living
  • Development of coping strategies for patient and family when skills cannot be restored
  • Recommendations for continued rehabilitation after acute rehab objectives have been met

The Stroke Program: An acute rehabilitation program whose focus is to provide comprehensive rehabilitation services for individuals after stroke. The program is designed to minimize and prevent chronic disability while helping to restore physical and emotional health, to increase independence in self care, and to increase participation in home, work, and community activities.

The General Rehabilitation Program: An acute rehabilitation program specifically designed for individuals who are medically stable with general rehabilitation needs due to orthopedic conditions, multiple trauma, amputation, spinal cord injury, neuromuscular and/or musculoskeletal disease, or any other debilitating injury or illness which meets the established admissions criteria.

Rehabilitation treatment goals for each of the programs listed above are individually determined by the members of the rehab team with the input of the injured or ill person and/or the designated family or caregiver support system. These individualized patient goals will focus on a combination of the following areas:

  • Strength
  • Functional Mobility
  • Activities of Daily Living
  • Safety
  • Communication Abilities
  • Cognitive Performance
  • Visual Perceptual Skills
  • Sexuality
  • Nutrition and Diet
  • Health and Wellness
  • Leisure Skills
  • Vocational and Educational Re-entry
  • Community Re-entry
  • Driving
