While disabled from working, many of our patients ask the doctors to assist them in the completion of special forms which cover credit card or car payments, mortgage payments, short term disability income, etc.
Our staff is willing to assist you with these needs. Please understand these requests are numerous, and the forms themselves are quite time-consuming.
A payment of $35-$50 per form (depending on length and subject to change) is required for completion, and a two- to three-week processing period is required.
A check payable to The Emory Clinic must accompany the form to be filled out. If you are mailing forms to us, please address them to the attention of your doctor's nurse. |
