Please do not hesitate to call us either through our call center at 1-888-WINSHIP (1-888-946-7447) or refer to our physician referral directory for contact names and numbers for our medical faculty and staff.

Call 1-888-WINSHIP (1-888-946-7447) to speak to a Call Center coordinator to provide information about your patient and his or her diagnosis. They will give information you have provided to the appropriate New Patient Referral Coordinator, who will call your office back within a few hours. You will be asked to FAX to us the following information at 404-778-1930.

• Pathology reports
  • Tissue
  • Bone marrow biopsy
• Lab reports
• XRAY and scan reports
• History and Physical
• Office notes
• Treatment records
  • Chemotherapy
  • Radiation
• Operative reports
• Hospital discharge summaries
• Copy of your patient's insurance card, front and back
