Pre-Operative Registration

The Emory Clinic offers Fast Track registration that will cut your admission wait time. Please be sure to call at least one business day before your pre-operative appointment in order to qualify. Call 404-686-5270 or 800-640-9293 to register. On the day of your appointment, tell the admissions representative that you are a "Fast Track" patient. You will be asked for your confirmation number and to show your insurance card when you sign in.

Your First Visit

Please plan to arrive 30 minutes before your scheduled appointment time so you may fill out any applicable patient forms. Should you need to cancel or reschedule your appointment, please notify us as soon as possible by calling 404-778-5673.

What to bring to your first appointment:

Please arrive 15 – 30 min before your scheduled appointment time

Please obtain a referral from your referring physician

  • Medical records: Include notes from referring physician, notes from any related hospital visit, relevant x-rays on disc, scans, labs, pathology reports and/or slides
  • List of all medications you are currently taking
  • Make a list of symptoms, health questions or concerns
  • Completed patient forms
  • Referring, and primary care, physician’s name, address and telephone number
  • Insurance card and (if provided one) insurance confirmation number

The initial consultation will include an evaluation based on current symptoms or conditions along with a thorough medical history. Our physicians prefer to review medical records prior to your scheduled appointment date. If you are able to provide records in advance, please call 404-778-3712 to arrange sending those medical records.

If you are unable to send records in advance, please bring all medical records, related to your diagnosis, to your scheduled appointment. Please see the above checklist to verify what to bring.

After the initial visit, your Emory surgeon will develop a treatment plan in coordination with your referring physician.
