Appointment/Consultation Scheduling

Patients and referring physicians can call our Emory University Hospital Midtown office at 404-686-8143, or contact Emory HealthConnectionSM at 404-778-7777 or 800-75-EMORY for immediate access to information on Emory services and physicians. 

Additionally, physicians and medical personnel can find more details here about Emory's referral and consultation process as well as such areas as credentialing, CME, and Emory's Physician Liaison Team.

A financial assessment will be performed prior to confirming an appointment. The following information will expedite the scheduling process:

  • Referral from the primary care physician (if needed)
  • The treating physician's name, address and telephone number
  • Patient's name, address, telephone number, date of birth
  • Patient's social security number
  • Diagnosis and date of diagnosis
  • Insurance information

First Visit

The initial consultation will include an evaluation based on current symptoms or conditions and compilation of a thorough medical history. Patients should bring all medical records related to their diagnosis, including notes, slides, and films from scans. To make arrangements for these items to be reviewed before the scheduled appointment, please call 404-686-8143. In coordination with the referring physician, a treatment plan is developed after the initial visit.
