Transitions Senior Program
The Transitions Senior Program is an outpatient program for elderly adults (65 or older) who have emotional concerns that interfere with quality of life such as withdrawal, isolation, grief, depression and loneliness.
Seniors that are referred to the program will have a complete psychosocial assessment to ensure that the program addresses their needs. Assessment includes discussion of current problems, observable changes in daily activities, medical/emotional problems and issues that might be of concern to the patient. Following the assessment, a personalized treatment plan and schedule is established.
The Transitions Program allows seniors to participate in plannned activities and therapies such as grief and anger management, cognitive therapy, problem solving, coping skills, leisure planning and relaxation, and relapse prevention. Lunch is provided daily to participants and transportation is available free of charge within a 25-mile radius of the Transitions Senior Program facility.
