SOURCE program
The objective of the Wesley Woods SOURCE (Service Options Using Resources in Community Environment) Program is to provide enhanced health services to elderly patients with chronic and long-term care needs. SOURCE provides members with enhanced case management, comprehensive primary care and community support services for elderly individuals with chronic care and/or long-term care needs.
The SOURCE program is sponsored by the Georgia Department of Community Health and administered by the Wesley Woods Center in Atlanta. SOURCE currently serves clients through several metro Atlanta hospital systems.

The mission of SOURCE is to enable elderly individuals to reside safely in non-institutional settings with the assistance of community resources, including medical and case-management services.
The goals of SOURCE are to integrate primary, specialty and home-based care to eliminate fragmented care and service delivery. SOURCE also aims to reduce emergency room visit frequency, multiple hospitalizations and nursing home placement caused by preventable medical complications. SOURCE helps stabilize social and lifestyle factors that affect health status, quality of life and compliance and assist with providing continuous coverage. Additionally, SOURE reduces the need for long-term institutional placement and increases options for alternative community placement for senior patients.

In order to participate in SOURCE, an individual must:
• Have SSI Medicaid coverage or SSI Medicaid and Medicare (non-HMO) coverage;
• Be 18 or older with a significant disability;
• Reside in DeKalb or Fulton County; and
• Have a chronic condition that has been present for at least three months.
During SOURCE enrollment, a case manager will visit the individual at home to assess specific patient needs. A primary care physician will conduct a medical exam and the case management team will create and oversee a plan of care to ensure each SOURCE member’s needs are being met.
