To Refer a Patient for heart failure or consultation, chose one of the following:
Call the Center for Heart Failure Therapy directly at 404-778-5273. Your staff will be asked to provide information specified on this form (PDF 60KB).
OR
Call the Emory Physician Consult Line 1-800-22-EMORY or 404-778-5050
Fill out this online form. Once complete, you will connect with an Emory Registered Nurse (RN), who will page an Emory Heart Failure specialist. You will hear back within minutes.
To Transfer a Patient from another hospital please call 404–778- 4930 or 1-800-43-HEART for an urgent evaluation and transfer. The Center's Transfer Service facilitates the transfer of patients into the Emory Healthcare system. Staffed 24 hours a day, 7 days a week by registered nurses and operators, the Transfer Service can assist you with any cardiac and/or vascular needs. With just one call, the staff will coordinate the transfer of patients into the cardiology, cardiovascular surgery and vascular services of Emory Healthcare
