Public Service Loan Forgiveness (PSLF) Form
The PSLF Form for current and former employees of Emory Healthcare, Inc is handled by the Employee Resources - Benefits department.
- Active Employees: will need to log onto: https://emoryhealthcare.tuition.io/
- Former employees & PRNs: Submit a completed and signed page 1 of the PSLF form along with page 2 is to be emailed to: ehc.hr.benefits@emoryhealthcare.org
Human Resources will complete page 2 of the PSLF Form and return it via email to the requester.
