Office Notes, Discharge Summary, History & Physical, Operative Reports, Pathology Reports, Laboratory Reports, EKG Reports, X-Ray Reports, Films, Disks, Current Diagnosis and any other pertinent information.

The Service Specialist will work with Financial Services to complete an estimate of the patient's cost for their visit. The Service Specialist will review the estimate with the patient and a deposit will be required.
Once the records have been reviewed and a deposit is collected, an appointment will be scheduled and an appointment letter will be generated. The letter will provide the location, doctor's name, date and time of the patient's appointment.
*Note: The appointment letter can be used by the patient to complete their visa application.
The patient is responsible for their own travel arrangements but our Service Specialist are happy to help with recommendations.
