Patient-Centered Primary Care Clinic
The Emory Patient-Centered Primary Care Clinic is a practice that aims to improve the overall health care experience by creating a new care model that emphasizes personalized, evidence-based medicine and greater coordination of care. Our program features include:
- Partnership with the patient and their family
- Personalized care based on each patient’s individual needs
- Comprehensive, team-based care to meet the patient's physical and mental health care needs, including prevention and wellness, acute care and chronic care
- Coordinated care between providers, specialists and other healthcare settings such as the emergency room
- Enhanced access to care by telephone, secure email and same-day or next-day appointments
What we do
We are a team of healthcare professionals devoted to providing general healthcare needs, including the following:
- Annual physical (or preventive) exams
- Caring for you when you are sick
- Helping you take care of any ongoing and complex illnesses such as diabetes and high blood pressure
- Focusing on prevention of illness
- Coordinating your care to keep you healthy

*At this time eligible patients include Emory employees and their adult family members covered under Emory’s self-insurance program administered by Aetna, as well as Aetna Medicare Advantage HMO/PPO members.
