Become a Patient

Patients may be self-referred or referred by another physician. The process is the same for all referrals and is outlined below:

Scheduling

For all appointments call 404-778-5050. PH Program Clinic visits are only available at The Emory Clinic (TEC) Clifton Rd location at this time (Clinic Building A). Please allow 2wks for scheduling, wait times are shorter or longer based on availability. Existing patients with urgent needs can typically be accomodated within a few days. For emergencies, dial 911. Please call 24hrs in advance if you need to cancel your appointment.

Clinic Visits and Parking

Please arrive 30 minutes early for your initial visit to allow us time to record your insurance information and for completion of a new patient questionnaire. Bring a valid form of identification, insurance information*, relevant medical documents (see below) and the completed New PH Patient Questionaire (if you received one in the mail). Valet parking is available and wheelchair service is provided free of charge. Water, coffee and refreshments are available in our waiting room, there is a cafeteria onsite as well.  

*Note: lack of valid insurance does not preclude evaluation in our PH clinic.

Relevant Documentation - Important, please read:

Provide all relevant medical documentation prior to your initial visit. This is to expedite your evaluation, prevent duplication of expensive testing and allow for comparison when new studies are required. Documents can be obtained through the medical records department of any office or hospital, typically at no additional charge to you. Federal HIPPA law mandates the signing of a records release, which can be faxed to any outside medical records department. Specify on the release that you would like records faxed or mailed to the Clifton Rd office (contact info above).

Documents to include:

1. Office or hospital notes pertinent to your complaint or condition, including a note from your referring physician if available.

2. Pulmonary function tests, new and old testing as far back as possible.

3. Lung imaging reports - Chest x-rays, CT scans, VQ scans, pulmonary angiograms, abdominal ultrasounds, lower extremity ultrasounds, and any other relevant imaging.

4. Lung imaging CDs - the actual images for the studies above help us with your PH evaluation and are required in some cases.

5. Echocardiographic data  - reports for ultrasound studies of your heart.

6. Right and Left Heart Cath reports - if possible, please have the pressure tracings from your right heart cath sent. These may be available through your cardiologist's office.

7. Laboratory and pathology studies - recent common lab testing, including rheumatological studies and recent HIV test, if available.

8. Referring physician contact information, including fax and office phone numbers.

As a general rule, the more documentation you provide the better we are able to help you. Be prepared to answer questions regarding your personal and family medical history, social and exposure history and past/present medication regimen.

Patient Portal

We are excited to announce the launching of the Emory Patient Portal, which allows patients on-line access to their diagnostic information including labs, radiological testing and more. You may also email office staff with questions regarding your care. Enrollment is easy, please ask your receptionist for a brochure with instructions and additional information.
