We offer organ transplantation for heart, kidney, liver, lung, pancreas, islet and hand. To refer your patient into a specific organ program, please use the drop down menu below for referral information and forms.

Kidney Transplant

To refer a patient for Kidney Transplant evaluation or to speak with a member of our staff, call 1-855-EMORYTX (366-7989).

Please download and complete a Kidney Transplant Referral Form and fax it to 404-727-8972. After we receive your referral, we will fax you a confirmation letter. All patients must take a transplant education class to continue the evaluation process. A letter will be mailed to the patient informing them of how to access our on-line education modules.

Liver Transplant

To refer a patient for Liver Transplant evaluation or to speak with a member of our staff, call 1-855-EMORYTX (366-7989).

Please download and complete a Liver Transplant Referral Form and fax them to 404-712-5811. All patients referred for liver transplant evaluation will be reviewed by the liver transplant team.

Lung Transplant

To refer a patient for Lung Transplant evaluation or to speak with a member of our staff, call 1-855-EMORYTX (366-7989).

Please download and complete a Lung Transplant Referral Form and fax it, to 404-727-1516. Faxed referral information will be reviewed by a transplant pulmonologist, and your patient will be scheduled for a consultation. If your patient has contraindications to transplantation, the transplant pulmonologist will contact you to discuss the patient’s case.

Heart Transplant

To refer a patient for Heart Transplant evaluation or to speak with a member of our staff, call 1-855-EMORYTX (366-7989).

Pancreas Transplant

Islet Transplant

To refer a patient to the Islet Transplant program or to speak with a member of our staff, call 1-855-EMORYTX (366-7989).

The Emory Transplant Center Clinical Research Program will ask you a series of questions related to the candidate's general health and diabetes history. If the candidate meets the screening criteria for inclusion into an islet research protocol, we will send a patient education booklet and application. After we receive the candidate's application we will review it with the islet transplant team.

Hand Transplant

To refer a patient to the Hand Transplant program or to speak with a member of our staff, call 1-855-EMORYTX (366-7989).

Hand Transplant is currently under clinical trial status and is open to men and women between 18 and 55 years of age with below-the-elbow amputation of one or both hands. Candidates must be able to give informed consent.
