Please make sure to have the following information available when calling:
• Date of birth
• Name, address and telephone number
• Social security number
• Insurance information
• Referral from your primary care physician (if surgery is medically necessary)
• Referring physician's name, address and telephone number
Appointments can be expedited by completing the patient pre-registration form (PDF 120 KB) prior to your first visit. Please print and fill out the form and bring it with you to your appointment — forms cannot be submitted electronically.
What to Expect From Your First Visit
In coordination with your referring physician, your first visit will include the development of a treatment plan based on your individual medical needs.
To ensure the most effective treatment plan, please bring all pertinent medical records, including recent radiology studies or films, to the first appointment. In particular, hard copies of recent angiograms and computed tomography (CT) scans can be essential in fully assessing your condition.
Do not assume your referring physician has already sent your records to our office unless we have verified their receipt.
