Providing Enhanced Health Services to Individuals With Chronic and Long-Term Care Needs
Service Options Using Resources in Community Environments (SOURCE) provides enhanced case management, comprehensive primary care and community support services to individuals with chronic care and/or long-term care needs.
Mission
The mission of SOURCE is to enable individuals to reside safely in non-institutional settings with the assistance of community resources, including medical and case-management services.
Goals
- Integrate primary care, specialty care and home-based care to eliminate fragmented service delivery;
- Reduce emergency room use, multiple hospitalizations and nursing home placement caused by preventable medical complications;
- Stabilize other social and lifestyle factors that affect compliance, health status and quality of life;
- Assist with providing continuous coverage; and
- Reduce the need for long-term institutional placement, and increase options for alternative community placement.

Eligibility
An individual must:- Have SSI Medicaid coverage or SSI Medicaid and Medicare (non-HMO) coverage;
- Be 18 or older with a significant disability;
- Reside in DeKalb or Fulton County; and
- Have a chronic condition that has been present for at least three months
Enrollment Process
- A case manager will visit each eligible individual at home to assess specific needs.
- A primary care physician will conduct a comprehensive medical exam of each SOURCE client.
- The case management team will establish and monitor a plan of care to make sure each SOURCE member's needs are being met.
The SOURCE program is sponsored by the Georgia Department of Community Health and administered by the Wesley Woods Center, located three miles from Emory University Hospital. SOURCE currently serves clients through several metro Atlanta hospital systems.
