Referral Check List

If you have been referred to the neurosarcoidosis clinic for evaluation, please work with your provider to fax the following information to  404-727-2532 (ATTN: Neurosarcoidosis Clinic):

To refer a patient with neurosarcoidosis for evaluation, please provide the following information via fax to 404-727-2532 (ATTN: Neurosarcoidosis Clinic):

  • Clinic visit notes from your neurologist and other sarcoidosis providers
  • Any pertinent documents related to hospitalization for neurosarcoidosis attacks or complications
  • Imaging reports of MRIs (brain, orbits, spinal cord) and body CT scans (chest, abdomen, pelvis)
  • Pathology reports of the biopsies taken to confirm sarcoidosis
  • Blood work results
  • Cerebrospinal fluid (CSF) testing results

Preparing for Your Appointment

Please provide a copy of your medical records and imaging results before to your initial visit with the Emory Neurosarcoidosis Clinic. This will allow the team to better understand your history with the disease and provide us with the opportunity to focus the visit on the most important matters for your care. The referral materials listed are necessary to confirm your diagnosis, understand the full scope of how sarcoidosis affects your nervous system, and provide clarity regarding treatments previously administered. In addition to imaging reports we would like to review the images from any MRIs or CTs that you have undergone for evaluation of neurosarcoidosis. Please mail your image disc or drop it off at our location below.

Day of Appointment

Please arrive at least 15 minutes early to your appointment to allow for check-in.
