- By participating in a Medicare Shared Savings Program, EHN Advantage looks to make healthcare choices more patient-centered and easier to navigate.
- Patients with serious health conditions see an average of 11 doctors, resulting in poorly coordinated care that is fragmented, costly and burdensome to the patient. EHN advantage makes healthcare more sensible and easier to navigate.
- By coordinating care through physicians in the Emory Healthcare Network, EHN Advantage ensures that quality of care remains high.

EHN Advantage employs a Care Coordination team that is specially trained in areas that affect seniors. Your team then works with your primary care physician to create an individualized care plan for you.
