Emory Healthcare conducts a community health needs assessment every three years to help us establish priorities for our community health improvement efforts. We are seeking input from both individuals who live in the community and individuals representing organizations who have an interest in the health and wellbeing of our communities.

The survey is in two sections. The first section is for individuals to share their personal view of community health needs. The second section is for people responding on behalf of an organization or constituency such as a faith-based or civic organization or municipality or government agency. If you represent an organization or constituency, please answer both sections of the survey. The survey will take approximately 10 minutes.
