New Patient Appointments

To Schedule new patient appointments with any of the physicians at The Emory Clinic Division of Infectious Diseases at Emory University Hospital Midtown, call 404-686-8114. Our clinic will need to review you medical records including visit notes and labs prior to scheduling your first appointment with an Infectious Diseases specialist.

Routine Appointments

To schedule routine appointments with your physician, call 404-686-8114. Same-day appointments may be available for sick visits. If your physician is not available, another physician, or our Nurse Practitioner or Physician Assistant may see you.

We recommend scheduling routine visits four to six weeks prior to the desired appointment date. Please arrive 15 minutes early for all appointments to allow time to complete required paperwork. We would also like to give you a friendly reminder to plan for traffic and parking delays.

Pre-Registration

As a new patient to The Emory Clinic, you will be asked to provide personal and insurance information. Please be prepared to provide this information when you schedule your appointment or we may contact you by telephone prior to your visit. If you are already an established patient and your information has changed, please arrive for your appointment at least 30 minutes early to allow us to update your information.

For your convenience, we have attached the forms you will be asked to complete and sign when you arrive for your first appointment. You may print these forms and complete them in advance if you like. Doing so will save time and paperwork for you on the day of our appointment. The first form is the Pre-registration Information. This form will need to be completed, signed and accompanied by your current insurance card if applicable. The second form is Initial History Form and should be completed by all patients. The final form is an HIV Review and will only need to be completed by HIV patients. If you have questions about the forms, leave the areas in question blank. We will go over the forms with you in detail when you arrive for your appointment.

Late Arrival: If you are running late for your appointment, please notify our office as soon as possible by calling 404-686-8114. If you arrive more than 15 minutes past your appointment time, we will try to accommodate you based on the schedule that day.

Cancellation Policy: When canceling a scheduled appointment, we request a minimum of 24 hours notice. This will allow us to accommodate the needs of other patients.

Pharmacy/Medication Refills: Please obtain medication refills during your visit with the physician. If you need medication refills and do not have an appointment, please notify us three to five days in advance of your refill needs. Ask your pharmacist to fax a request to 404-686-4841 with the following refill information: your full name, date of birth, medication, dosage, pharmacy telephone number and the name of your physician.

Test Results: To optimize your visit, have your routine labs drawn two weeks prior to your scheduled appointment. This will enable your physician to review your labs and discuss them with you in person and respond to any questions you may have at that time. For labs drawn on the day of your visit, please allow two weeks for your clinic staff to review the results with you. If you do not hear from our clinic within two weeks, please call the nurse at 404-686-8114.
