Become a Patient

Emory is a leading academic institution.  Patients who receive care at the Emory Pain Center will likely be seen by physicians in specialty training, as well as the attending physician. This presents a unique opportunity for our patients to receive a thorough evaluation by leading experts in the field of interventional pain medicine.  We look forward to partnering with you to design your personalized treatment plan.

To make the most of your visit, our pain specialists will need to review any prior medical records relevant to your current complaint from the last 3 years. Please request for them to be faxed to 404-686-4475 prior to your initial visit.

Please Bring To Your First Appointment:

  • Your insurance card and photo ID; a referral if required by your insurance
  • Completed Patient Forms (New Patient Questionnaire, Acknowledgements of No Show & Medication policy)
  • List of all current medications.
  • All prior imaging (film, CD, and reports) performed within the last 3 years related to your current complaint. 

During Your Visit:

Our average new patient visit lasts between 60-120 minutes, depending on the needs of each patient. Please arrive at least 15 minutes before your scheduled appointment time.

Should you need to cancel or reschedule your appointment, please provide 24 hours’ notice. Our scheduling team can be reached at 404-686-2410.

During your initial visit, our pain specialists will create a customized treatment plan that includes an optimal combination of therapies to provide you with safe, long-term relief and allow you to be more active in your life.  

*Please do not expect a narcotic prescription on your initial visit.
