Interested in undergoing an evaluation for your symptom of excessive daytime sleepiness?

In order to assess the appropriateness and potential benefits for you or your family member, we must first review your medical records. Due to the unique administrative and physician time and detail associated with this review, a $200 up-front, non-refundable administrative fee will be issued.

  2. New Patient Sleep Questionnaire
  3. New Patient Sleepy Packet

Once completed, please send the forms, as well as all pertinent sleep-related medical records (see list below), in scanned email to sleepcenter@emoryhealthcare.org or fax to 404-712-8145 with “Hypersomnia Record Review” listed as the subject. Please allow 30 days for this review process, which includes identifying potential dates for your visit and evaluation. Note: records not labeled “Hypersomnia Record Review” or incomplete forms and inadequate records will cause a delay in processing. Below is a listing of sleep-related medical records:

  • All nocturnal polysomnograms (sleep studies)
  • All multiple sleep latency tests (daytime napping tests)
  • Listing of all attempted treatments for your sleep disorder and to what extent they were effective
  • Physician notes documenting inadequacy of conventional treatments
  • Physician notes documenting treatment progress for any psychiatric diagnosis (if applicable)

Frequently Asked Questions

  • Patients who are invited for an appointment are typically evaluated over the course of two days.
  • Day 1: initial intake visit with the physician, which includes vigilance testing, serum/blood laboratories and consent forms.
  • Day 2: follow-up appointment to review available blood labs, establish a care plan for when you return home and a lumbar puncture if deemed appropriate by the Emory Sleep Center physician.

Hypersomnia visits are scheduled on Thursdays and Fridays.

Yes, it is important to maintain care with your local provider after your appointment at the Sleep Center for medication maintenance, paperwork requests and routine clinic visits.

Currently, the Emory Sleep Center for Hypersomnia bills clinic visits, lumbar puncture procedure (if applicable) and laboratories to your health insurance provider. It is your responsibility to verify your benefits coverage prior to your visit.
