To report adult cardiac arrest or life-threatening medical conditions, call the Emergency Number specific to your location and say, "Medical Alert Blue Adult". Give the exact location and any additional information requested.

Pregnant or postpartum patient in distress.
To report a pregnant or postpartum patient in distress, call the Emergency Number specific to your location and say, "Medical Alert OB". Give the exact location and any additional information requested.

To report a need for a stat cesarean or vaginal delivery, call the Emergency Number specific to your location and say, "Medical Alert Delivery." Give the exact location and any additional information requested.

Rapid Response.
To get medical assistance for patients, visitors or staff exhibiting warning signs of deterioration or instability, call the emergency number specific to your location and say, "Medical Alert Rapid Response". This code can be called by anyone.

This code is called for any inbound patient or hospital inpatient for whom there is a clinical suspicion of sepsis or septic shock. Call the Emergency Number specific to your location and say, "Medical Alert Sepsis", give the exact location and any additional information requested.

ST-Segment Elevation Myocardial Infarction - Only called by Designated Individuals.
This code is called for any inbound patient or hospital inpatient that have ST-segment elevation on their 12 lead EKG to activate the designated response team. Call the Emergency Number specific to your location and say, "Medical Alert STEMI", give the exact location and any additional information requested.

Inpatient is suffering from an anatomical medical condition that prohibits them from being intubated or oxygenated, therefore requiring a surgical intervention to open the airway. Call the Emergency Number specific to your location and say, "Medical Alert Surgical Airway". Give the exact location and any additional information requested.

ACTIVE THREAT emergency code icon

Active Threat

Person with a weapon is acting hostile or making threats.
Indicates a person with a weapon (firearm, knife, etc.) who is acting hostile, making threats or shots have been fired. Ensure 911 has also been contacted. Run, Hide, Fight. Avoid the area.
BOMB THREAT emergency code icon

Bomb Threat

Bomb threat.
Bomb threat, if received by phone:
  • Obtain as much information as possible, "DO NOT HANG UP", call public safety and inform them of the bomb threat, give your exact location and any additional information requested.
  • Complete the bomb threat card or form. Write down everything you can remember concerning the caller; voice, accent, background sounds, etc.
If received by mail:
  • Do not touch the package. Call public safety and give your exact location and any additional information requested.
Make sure to:
  • Notify your supervisor immediately.
  • Be prepared to search your own area to identify unusual objects.
  • Call public safety first if an object is found.
If at an ambulatory site, call 9‑1‑1.

Access to a unit or facility is restricted.
Indicates there is an emergency situation or event in (or immediately outside of) a unit or facility that threatens the life‑safety of patients, staff, and visitors inside.

Missing Person

Missing patient bedded area.
Missing Patient (Adult or child) or Visitor (Adult, Child, or Non‑bedded Infant): Using department specific search procedures, search for the missing patient or visitor. Assigned staff to monitor exit/access points, as appropriate.

FIRE ALARM emergency code icon

Fire Alarm

To report an actual fire or fire drill.
To report a Fire follow the acronym: R.A.C.E.

  • Rescue anyone in immediate danger and reassure patients and visitors.
  • Alarm by pulling nearest FIRE ALARM box and call the emergency number specific to your location.
  • Confine the fire by closing doors and shutting off oxygen supplies according to hospital and department policy.
  • Extinguish or evacuate, if possible.
For fire extinguisher use, follow the acronym P.A.S.S.
  • Pull the pin.
  • Aim at base of the fire.
  • Squeeze the handle.
  • Sweep from side to side.
HAZARDOUS SPILL emergency code icon

Hazardous Spill

Hazardous material spills.
If a Small Hazardous Material Spill implement department‑specific spill procedures. For Large Hazardous Material Spills call the emergency number specific to your location and say "Facility Alert Hazardous Spill", give the exact location and any additional information requested, implement department specific procedures for:

  • Confirm that required PPE and spill kit is in use
  • Leave the area, close the door (if possible) and restrict access
  • Ensure those exposed are given emergency medical care
  • Access and provide the Safety Data Sheet (SDS)
  • Notify your Supervisor and the Department of Safety Management

Internal Or External Disaster
Only a designated leader on duty can initiate "Facility Alert Mass Casualty (Internal or External)".

Staff on Duty:
  • Implement Facility Alert Mass Casualty procedures and follow ICC direction
  • Implement department specific plans and report status to the ICC
Staff NOT on duty:
  • Verify staffing requirements with your department (do not call ICC)
  • Be prepared to report in and remain for the duration of the disaster

Tornado Watch indicates a tornado is possible. Close drapes and blinds. Inform patients and visitors to move away from glass.

Tornado Warning indicates a tornado has been confirmed or sighted. Prepare patients and visitors for sheltering as directed.

Hurricane Watch/Warning indicates a hurricane is possible or confirmed. Verify staffing requirements with your department.

Condition Snow/Ice indicates an ice or snow emergency is likely.
