Effective immediately, to refer a patient to IV Therapy, an Emory Healthcare medication order set must be completed. If an medication order set is not listed below, please contact the department directly for further instructions.
Please fax the completed Emory Healthcare order set along with the following documents to 866-439-0210:
- Patient demographics, including active insurance information
- Clinical documentation supporting medical necessity (e.g., office notes, lab results and radiographic studies, when applicable)
- Required lab orders and/or results for treatment
