IV Therapy Referral Options
The process for submitting IV Therapy referrals depends on whether the ordering provider is credentialed with Emory Healthcare.
- Credentialed Emory providers are required to submit IV Therapy orders electronically using Epic Hyperspace.
- Non-credentialed community providers may continue to refer patients using the Emory Healthcare medication order sets available on this page.
Patient Referrals
For non-credentialed community providers, an Emory Healthcare medication order set must be completed and submitted when referring a patient for IV Therapy.
Please fax the completed Emory Healthcare order set to 4787-322-4817. Each referral should include:
- Patient demographics, including active insurance information
- Clinical documentation supporting medical necessity (e.g., office notes, laboratory results, radiology reports, when applicable)
- Required laboratory orders and/or results needed for treatment
- If the medication order set you need is not listed below, please contact the department directly for assistance.
