The model helps families navigate the long road of dementia due to Alzheimer’s disease, frontotemporal degeneration, Lewy body disease, cerebrovascular disease, and other causes.
In 2016, the Integrated Memory Care Clinic earned Patient-Centered Medical Home Recognition from the National Committee for Quality Assurance (NCQA).
