Access to clinical staff is very important to the families we serve at Integrated Memory Care (IMC). The clinic team answers calls and responds to requests as soon as possible. We also have reserved same-day appointments for acute health concerns. Our comprehensive care model allows patients to have longer appointments with the clinic nurse practitioners.
IMCC After-Hours: For urgent health concerns that arise when the clinic is closed (after 5 p.m. on weekdays, weekends, or holidays), caregivers can call the after-hours line and speak with an IMC nurse practitioner. The after-hours line is a cell phone and is not a secure form of communication.
MyChart: Patients and, with certain permissions in place, caregiver and family members may securely correspond with the IMC, request prescription refills, check lab results, and view your health care records via MyChart.
Prescription Requests and Refills: Requests for prescription refills can be obtained by calling 404-712-6929 during regular clinic hours. Requests can also be made via MyChart.
Integrated Memory Care Clinic Patient and Family Advisory Committee: Patients and family members are invited to become members of the IMC team and collaborate with the clinic as Patient and Family Advisors (PFAs). Our PFAs provide valuable feedback that helps to improve our patients’ experience. Meetings are typically held on a monthly or bi-monthly basis.
