Lily Phan, NP

I am a Family Nurse Practitioner with over eight years of experience providing primary care to patients of all ages. I chose a career in healthcare because I enjoy building relationships with patients and making a meaningful difference in their health and well-being. I especially value getting to know my patients, listening to their concerns, and working together to develop practical, personalized care plans.  I wish I could tell every patient that you do not have to wait until something is seriously wrong to take care of yourself.  I am passionate about preventive care, health education, and helping patients feel empowered to take an active role in their health. I enjoy the variety of primary care, from managing chronic conditions and addressing acute concerns to supporting patients through different stages of life.  During my graduate education, I completed academic research and evidence-based projects focused on improving patient care and clinical outcomes.  While I have not had research formally published, these experiences strengthened my ability to critically evaluate clinical evidence, apply research findings to practice, and use evidence-based guidelines when caring for patients.  
My family moved here from Vietnam in 2004, and we have lived here since.  I like the city's diversity, a multicultural environment, welcoming communities, and strong sense of opportunity.  Outside of work, I enjoy spending time with my family, traveling, and trying new foods. I love designing and decorating my home.  I recently moved to Cumming, GA.  I am married.  We have one boy and a girl on the way.  
I am excited to be part of the Buford community and look forward to caring for patients and families in the area.
