For virtual telemedicine visits, please complete the forms prior to your appointment and fax them to 678-843-5479 or attach them to your secure message through the Emory Patient Portal prior to your appointment so that you can go over them with the clinical team. If you do not have an Emory Patient Portal account, you will need to request an email invitation by calling 404-727-8820.
