Support Pharmacy Fellowship Application

Fellowship applications are accepted by email. (You DO NOT need to enroll in the ASHP matching program.) Your application is complete if we receive all required documents before the March 31st deadline. Please email all required documents to vivian.zhao@emoryhealthcare.org with NSPF and your name in the subject line.

 

Required Documents

• Completed application
• Transcripts
• Curriculum Vitae - Please include all schools and universities attended, degrees conferred or anticipated, employment history, honors and awards, rotation experiences, research or teaching experiences, extracurricular activities, and other pertinent information.
• Letter of Intent - Please state your reasons for pursuing nutrition support pharmacy fellowship training and why you are interested in the nutrition support pharmacy fellowship program at EUH.
• Short and long-term goals
• Three recommendations: Please send the electronic form to those providing recommendations after completing the top portion of the form. Electronic submissions should be from the recommender’s institutional or business email address to verify authenticity. At least one of your recommendations must be from a clinical APPE rotation or residency preceptor.
